| 目的 探讨不同平面体外冲击波对兔糖尿病足溃疡创面愈合的影响。方法 选择体重2~3 kg的新西兰大白兔40只,采用高脂喂养、静脉注射四氧嘧啶溶液、股动脉结扎法建立糖尿病足溃疡模型。将新西兰兔随机分为4组(n=10):模型组,冲击波治疗1组在患肢近端给予体外冲击波治疗,冲击波治疗2组在创面周围给予冲击波治疗,冲击波治疗3组在患肢近端及创面周围给予体外冲击波治疗。治疗4周,1周2次。检测患肢创面愈合情况、血糖及患肢体温的变化。结果 与其他组比较,SWT2组和SWT3组溃疡愈合明显改善,患肢体温明显改善(P<0.05)。各组新西兰大白兔血糖水平无显著变化(P>0.05)。结论 体外冲击波治疗能促进糖尿病足溃疡的愈合,更重要的是创面周围联合患肢近端治疗效果优于局部治疗。 |

| Objective To investigate the effects of extracorporeal shock waves applied in different planes on the healing of diabetic foot ulcer wounds in rabbits.Methods Forty New Zealand white rabbits weighing 2-3 kg were selected to establish a diabetic foot ulcer model through high-fat feeding, intravenous injection of alloxan solution, and femoral artery ligation. The rabbits were randomly divided into four groups (n=10): a model group, a shock wave therapy group 1 (SWT1) receiving extracorporeal shock waves at the proximal end of the affected limb, a shock wave therapy group 2 (SWT2) receiving shock waves around the wound, and a shock wave therapy group 3 (SWT3) receiving extracorporeal shock waves at both the proximal end of the affected limb and around the wound. Treatment was administered twice a week for four weeks. Changes in wound healing, blood glucose levels, and body temperature of the affected limb in the rabbits were measured.Results Compared with the other groups, the SWT2 and SWT3 groups showed significant improvements in ulcer healing and a marked increase in the body temperature of the affected limb (P<0.05). There were no significant changes in blood glucose levels among the groups (P>0.05).Conclusion Extracorporeal shock wave therapy can promote the healing of diabetic foot ulcers. More importantly, the combination of shock waves applied around the wound and at the proximal end of the affected limb yields better therapeutic outcomes than localized treatment alone. |
