| 目的 比较儿童型和青少年型尤文肉瘤的临床特征和预后特点的差异。方法 收集美国SEER数据库中1975年至2016年确诊的1 389例6 ～ 18岁的尤文肉瘤患者的临床资料。根据发病年龄分为儿童组（598例）和青少年组（791例）。通过Logistic和Cox回归分析分别确定尤文肉瘤分期和预后的危险因素，利用生存分析探究儿童型和青少年型尤文肉瘤预后的差异。此外，利用Cbioportal数据库中的Curie数据集对儿童型和青少年型尤文肉瘤预后差异进行初步验证。结果 SEER数据库中，儿童组和青少年组尤文肉瘤的诊断年份、性别、接受放疗比率、病变部位、远处转移的比例、AJCC分期比较，差异有统计学意义（P＜0.05）。Logistic回归分析表明青少年型是尤文肉瘤分期较晚的危险因素，Cox回归分析表明青少年型是影响尤文肉瘤预后的重要因素。利用Cbioportal数据库中的Curie数据集初步验证，发现青少年型尤文肉瘤患者的总体死亡率高于儿童型患者。结论 相比于儿童型，青少年尤文肉瘤患者总体分期和AJCC分期更晚，预后也相对较差。 |

| Objective To compare the clinical features and prognosis of Ewing's sarcoma between children and juvenile.Methods The clinical data of 1 389 Ewing's sarcoma patients aged 6-18 years old from 1975 to 2016 in the SEER database of the United States were collected, and they were divided into two groups according to the age: Children group (598 cases) and juvenile group (791 cases). Logistic and Cox regression analyses were used to determine the risk factors for stage and survival in patients with Ewing sarcoma, and survival analysis was utilized to explore the difference of prognosis between childhood and adolescent. In addition, the Curie dataset from the Cbioportal database was applied to preliminarily verify the difference in prognosis between childhood and juvenile Ewing's sarcoma.Results There were significant differences in diagnosis year, gender, radiotherapy rate, lesion site, proportion of distant metastasis, AJCC stage between children and adolescents in the SEER database (P<0.05). Logistic regression analysis showed that juvenile type was a risk factor for late staging of Ewing's sarcoma, and Cox regression analysis showed that juvenile type was an important factor affecting the prognosis of Ewing's sarcoma. Using the Curie data set from the Cbioportal database, we found that the overall mortality rate of juveniles with Ewing's sarcoma was higher than that of children.Conclusion Compared with childhood Ewing's sarcoma, the overall stage and AJCC stages of adolescent Ewing's sarcoma are generally later, and the prognosis is relatively less favourable. |
