Objective The aim of this study was to compare the clinical efficacy of TightRope system versus clavicular hook plate for Neer IIb distal clavicular fractures. Methods Between Jun 2012 to Jun 2016, 37 patients with acute, closed Neer IIb distal clavicular fractures were treated by TightRope system (n = 19) or clavicular hook plate (n = 20) in our hospital. Baseline patient characteristics, Clinical and radiographic postoperative assessments at 3, 6 and 12 months postoperatively and complication rates were retrospectively analyzed. Results There was no significant difference in baseline characteristics, operation time, blood loss and wound healing between the two groups. In comparison, TightRope system group had smaller incision (P<0.01) and significantly higher CMS at 3 and 6 months postoperatively, especially in measures of pain, range of motion and activities of daily living (P<0.01). The CMS at 12 months follow-up revealed similar results between the two groups, but the pain score in clavicular hook plate group was still significant lower (P=0.02). All fractures healed in 6 months postoperatively and neither serious complications nor secondary operation was presented. Conclusion TightRope system is an effective and safe surgical technique for the treatment of Neer IIb distal clavicular fractures. It is characterized by minimally invasive, less morbidity and early rehabilitation. |
